Form 288a is a Companies House form which is used to appoint a director or secretary.
The PDF form requires that the name, address, date of birth, occupation and nationality of the appointee is given and that an existing director or secretary authorise the appointment by signing the document.
When appointments are made using an electronic Form 288a via an online system, an incumbent’s signature is not possible and therefore the new company officer is required to answer three from seven security questions.
The available questions are currently: telephone number, mother’s maiden name, father’s first name, passport number, national insurance number, town of birth and the colour of the appointee’s eyes.
Both electronic and PDF Form 288a also requires that the new director state other directorships which they currently hold or have held in the previous five years. Space is usually provided for five entries to be made, whilst additional positions can be continued on paper and posted with the PDF form or separately in the case where the appointment is being made online.
Form 288a permits a person or company which is being appointed to both director and secretary positions, to do so one the same document, simply by ticking the relevant boxes.
