Companies House Form 288b is used for the resignation of a company director and or secretary. Once completed, it should be sent to the Registrar of Companies or to a formation agent if instructed to do so by them.

Company Number
The company number can be found of the certificate of incorporation or via the web check facility at Companies House.
Date of Termination of Appointment
The relevant date the one on which the resignation of the director or secretary is to take place, that is, the effective date from which they will no longer serve the company
Resigning as director and secretary
It is possible to use one Form 288b to facilitate the resigning as both director and secretary. In such a case, both the appropriate boxes should be marked.
Signature
A remaining company officer (who is in place at the time the document is completed) must affix their signature to the form. The date field should be populated with the date of which they signed the document.

This information is not mandatory; however, it might expedite matters should the Form 288b contain a mistake. In the absence of any contact details, Companies House will liaise via the company’s registered office address.
Other Information
Note that the address of the resigning officer is not required as is the case on Form 288a when appointing a director or secretary.
DX number and DX exchange are document transfer facilities typically used by solicitors and other professionals. It is not necessary to fill in these fields if such details are not known.
